2024-2025 School Registration Medical forms for New and current students

You will find all required Medication forms in your Skyward Family Access registration information EXCEPT:

All Students requiring medication at school must print out, complete and sign the following forms:

1. The Request for Administration of Medicine ( One for each student requiring service) Form can be printed off or obtained from your building's school nurse as the medication form must be signed by a doctor and parent for your student to receive medication at school. PDF

2. Self administration of medicine Form for inhalers require the labeled prescription box. If you want your student to carry either of these with them, you must fill out and sign the self administration of medicine form. PDF
